Terms of Service

ACCURACY AND ACCESS

You can discuss your medical record with your treating clinician at any appointment. If at any time you feel a record on your file is not accurate, please advise the specialist for an amendment.

DATA MIGRATION

To ensure continuity of care, your patient file will be moved in entirety by the practice should it move to another practice management software provider in the future. By signing this document, you consent to your data being migrated by our staff if required.

TELEHEALTH

Your appointments may be scheduled face-to-face, via phone or via videoconference. Appointments will not be recorded unless discussed for a specific purpose. By engaging in service, you acknowledge your consent to participate in telehealth services and understand that refusal to engage in a service via telehealth may limit the availability of appointments.

HOW YOUR INFORMATION IS PROTECTED

Your medical record is held on a cloud based server that is password and two-factor authentication protected. It is bound by the Privacy Act.

CHANGES TO YOUR INFORMATION

Any changes to the information housed on your medical record will always be made as an amendment to the original file. The original document will remain.

You do not have to give all of your personal information, but if you do not this may impact the care provider and clinic’s ability to provide services to you.

MISLEADING INFORMATION

Our clinicians and clinic reserve the right to terminate services to a client in the event that misleading or omitted information leads the clinician to believe they are not suited to provide services to the client.

BEHAVIOUR AND CONDUCT

Our healthcare business has a strict policy of zero tolerance towards any form of abuse or harassment towards our staff or other patients. We believe in providing a safe and respectful environment for everyone, and any behavior that violates this principle will not be tolerated. We take every complaint seriously and will take appropriate action to address any such incidents, which may include termination of services. We encourage our patients to treat our staff and fellow patients with kindness and respect, and help us create a positive and supportive healthcare experience for all.

FEES

Each clinician within the clinic has an individual fee schedule. You may request the fee schedule for your upcoming appointments at any time should you request this. Please note that failure to request information regarding fees, does not excuse the client or guardian from their responsibility to meet the financial obligation of the service engaged. Fees are payable prior to or immediately following the session. Patient reports will not be issued without the finalisation of outstanding accounts. Patients will not have further appointments booked whilst an account has an outstanding balance due.

DOCUMENTATION COMPLETION

Any documents that you need to be completed should be brought to your consultation. Requests for document completion outside of a scheduled appointment will incur a fee at the standard hourly rate. An estimate for form completion may be requested during your consultation with the care provider.

PRESCRIPTION REQUESTS OUTSIDE OF A SCHEDULED CONSULTATION

Requests for repeat prescriptions (the common scenario is of a patient having run out of a medication, or having lost a script) will require a brief telehealth consultation of 15 minutes before this can be provided. This ensures that we comply with Medicare guidelines and good practice prescribing guidelines. The cost for this appointment is $80.00.

REBATES

Medicare: rebates are available on some appointments within the clinic. As the rebate available is dependent on the clinician, the appointment type and duration, we suggest you enquire as to the available rebate in your unique circumstance at the time of booking.

Private Health Insurance: If you have private health insurance that includes extras cover for psychology, you may be eligible to a partial rebate on some psychology services. It is recommended that you contact your insurance provider for further information about your individual coverage.

We are able to lodge your Medicare rebate claim for you on the day of the consultation. This is generally repaid by Medicare within 24 hours.

Rebates are not guaranteed by our clinic and are determined by the relevant funding body.

CANCELLATION POLICY

If you need to cancel or postpone an appointment, please provide at least 48 hours notice, otherwise you will be charged 50% of your appointment as a cancellation fee. Failure to attend an appointment without notice will attract a 100% failure to attend fee which is not eligible for rebate. In exceptional circumstances, this fee may be waived.

THIRD PARTY PAYMENTS

Patient’s that will have their consultation fees paid by a third party need to contact the practice in advance of your appointment to discuss this process. It is expected that consultations in these situations are paid in advance by the third party, or at the time of the consultation as per our usual process. We are unable to accept delay payments and apologise if this causes any inconvenience.
