Transferring your care

When transferring your mental health or disability care to Navigating Health, there are several steps involved. First, you can provide your current provider with written authorization to release your medical records to Navigating Health. Then, we will review your medical history and assess your current needs. We will make an initial appointment with you to get a complete picture of your health. Once we have all the necessary information, Navigating Health will work with you to develop a customized care plan that meets your individual needs and goals. Throughout the process, communication and coordination between your current provider and Navigating Health is essential to ensure a smooth transition of care.

Assessment process 

Structure & Inclusions 

Our Transfer of Care assessments include the following:

  • Screening of referral and intake forms
  • Administration of any pre-assessment questionnaires (if required)
  • Initial assessment session
  • Treatment planning and further assessment as required (costs to be advised as required)
  • A summary report including any updates to your treatment plan- this will assist in ensuring all professionals involved in your care are updated regarding the transfer of care and developments in your treatment.

Common questionnaires we may send during this process  

  • The Depression Anxiety Stress Scales-21 (DASS-21): is a concise self-report tool designed to measure the negative emotional states of depression, anxiety, and stress. The DASS-21 is widely used in both clinical and research settings for its efficiency and robust psychometric properties. It provides separate scores for each subscale as well as an overall score for general mental health, aiding in the identification and differentiation of these three common emotional states.

  • Other questionnaires to be determined by requirements listed on patient referrals.
