  • Have general concerns, but aren’t sure what is needed
  • Have been recommended to seek supports for their mental health
  • Have a disability or other chronic health condition and require ongoing supports
  • Would like to seek advice about next steps and investigations

Our general assessment appointments 

Our general assessments involve a comprehensive evaluation of multiple sources of information. This may include interviews with parents, significant others and teachers, as well as standardized tests and observation of the individual’s behaviour. The purpose of the assessment is to gain a better understand the presenting condition and determine the most appropriate treatment moving forward. It’s important to note that the assessment process can vary depending on the age of the individual, as well as their needs and circumstances.

Our standard assessment package includes a brief diagnosis and treatment letter. If you are wishing to apply for NDIS funding, a more in depth report is recommended. Our staff will book time for an in depth report when you first make your booking. If it is decided this isn’t needed, we will cancel the booking after your first session. We always prefer to book you in and ensure your place is reserved to reduce delays to progressing with supports.

Our General Health assessments include the following:

  • Screening of referral and intake forms
  • Administration of pre-assessment questionnaires
  • Initial assessment session
  • Treatment planning and further assessment as required (costs to be advised as required)
  • Summary report including assessment findings, diagnosis and treatment plan
  • School/employment letter with inclusive recommendations

Common questionnaires we may send during this process  

  • The Depression Anxiety Stress Scales-21 (DASS-21): is a concise self-report tool designed to measure the negative emotional states of depression, anxiety, and stress. The DASS-21 is widely used in both clinical and research settings for its efficiency and robust psychometric properties. It provides separate scores for each subscale as well as an overall score for general mental health, aiding in the identification and differentiation of these three common emotional states.   

  • Other questionnaires to be determined by requirements listed on patient referrals.
